1. Standard Form of Application (Assam Gazette Part-IX).

2. Two recent passport-size photographs.

3. Original Documents & Self-Attested Photocopies:

• Age Proof (HSLC Admit Card).

• All Marksheets and Pass Certificates.

• Work Experience Certificates.

• Identity Proof (Voter ID / Aadhaar).

• Caste/PWD Certificate (if applicable).
